    养老服务资源的空间分布情况关乎国家养老体系整体效能。运用ArcGIS软件观测全国地级市养老服务资源分布,发现其分布高度拟合“胡焕庸线”,呈现显著的空间正相关与群聚特征,并形成五大高密度核心带。针对现实中养老服务资源分布与老龄人口之间存在的“空间错配”现状,在通过热力空间测度刻画供需匹配关系的基础上,采用截面OLS回归结合城市级聚类稳健标准误处理,深入检证影响资源空间配置的深层经济社会机理。实证研究表明:第一,人口老龄化程度对养老服务资源集聚具有基础拉动作用,但该机制高度依赖社会保障覆盖水平与居民收入的“条件触发”;第二,发达地区养老产业存在“成本病”挤出效应,高昂的地租与运营成本使得部分经济高地未能实现基础养老床位的等比例扩张;第三,生活环境舒适度与医疗资源丰富度正成为重塑养老空间网络的核心驱动力,且区域需求异质性显著影响了养老服务资源的质量结构。基于上述机理,提出优化养老服务资源分布的实质性对策:一是针对“医养分离”痛点,探索“医养床位”动态转换与医保无缝结算机制;二是应对大城市“成本病”,推行养老补贴跨域结转,依托周边节点城市打造“康养飞地”;三是直面城乡空间断裂,实施财政转移“空间倾斜”,构筑“县-镇-村”三级普惠网络,以切实弥合农村养老的“空间塌陷”。

    Abstract:

    The spatial distribution of elderly care service resources is crucial to the overall efficacy of the national eldercare system. Utilizing ArcGIS to observe the distribution of elderly care service resources at the prefecture-level city scale across China, this paper finds that the distribution highly aligns with the demographic pattern of the “Hu Line”. It exhibits significant positive spatial autocorrelation and agglomeration characteristics, forming five high-density core belts. Addressing the current “spatial mismatch” between the distribution of elderly care service resources and the aging population, this paper first delineates the supply-demand matching relationship via spatial thermal analysis. Subsequently, it employs cross-sectional OLS regression coupled with city-level clustered robust standard errors to thoroughly investigate the underlying economic and social mechanisms driving spatial resource allocation. Empirical results indicate that first, population aging exerts a fundamental driving effect on resource agglomeration; however, this mechanism is highly contingent upon the “conditional triggering” of social security coverage and residents’ income levels. Second, this paper reveals a “cost disease” crowding-out effect within the eldercare industry in developed regions, where exorbitant land rents and operational costs prevent certain economic hubs from achieving a proportional expansion of basic care beds. Third, environmental amenities and the abundance of medical resources are emerging as core driving forces in reshaping the spatial network of elderly care services, while regional demand heterogeneity significantly impacts the qualitative structure of these resources. Based on the aforementioned mechanisms, this paper proposes targeted optimization countermeasures: first, to address the pain point of “medical-elderly care separation”, it explores dynamic conversion mechanisms between “medical beds” and “elderly care beds”, alongside seamless medical insurance settlements. Second, to counteract the “cost disease” in megacities, policies should promote the cross-regional portability of eldercare subsidies and establish “wellness enclaves” in peripheral node cities. Third, confronting the urban-rural spatial divide, financial transfer payments should implement a “spatial tilting” strategy to construct a three-tier “county-town-village” inclusive service network, thereby effectively bridging the “spatial depression” in rural elderly care services.
